¿Qué es la Radioterapia?
La radioterapia consiste en la aplicación de radiaciones ionizantes como tratamiento de diversas enfermedades neoplásicas. Se puede administrar como tratamiento único o coadyuvante con quimioterapia y cirugía. Puede administrarse con fines curativos en diferentes enfermedades (cáncer de cérvix uterino, cáncer de mama, cáncer de próstata, cáncer de cabeza y cuello, cáncer de recto, etc) o con fines paliativos (tratamiento del dolor, síndrome de vena cava superior, hemostática, compresión medular).
¿Cómo actúa la radioterapia?
La forma de actuar de la radioterapia es impidiendo el crecimiento o división de células malignas incluso puede destruir las mismas. Se sabe que las células tumorales crecen y se dividen más rápido que las células sanas, pero también son más sensibles a la radiación y no son capaces de reparar el daño causado por la misma; esta es la base biológica principal por la que se trata estas enfermedades con radioterapia.
¿Qué tipos de radioterapia existen?
En la práctica diaria las radiaciones utilizadas pueden provenir de fuentes naturales como C060 o ser generadas de forma artificial como los rayos X. Existen dos tipos básicos de radioterapia: la teleterapia o radioterapia externa o a distancia donde se utilizan unidades de cobaltoterapia o aceleradores lineales y la fuente está a cierta distancia del paciente y la braquiterapia o radioterapia intracavitaria o de contacto donde se aplica el material radioactivo en contacto con el tumor (intersticial) o por orificios (intracavitaria).
En radioterapia la unidad de medida es el Gray (Gy), que es la energía absorbida por unidad de masa, la dosis total utilizada varía según la intención del tratamiento (curativo o paliativo) de 8 a 70Gy, repartidos en dosis diarias de 1,8 a 2Gy en la teleterapia y dosis semanales en la braquiterapia.
¿Cómo es el proceso de radioterapia?
El proceso del tratamiento con radioterapia externa implica varias fases, en primer lugar el paciente es valorado por el especialista para ver si es o no candidato a radioterapia, posteriormente se realiza la simulación donde el medico define el volumen de tratamiento que consta del lugar de la lesión con varios centímetros de margen, se realizara la verificación de los campos por medio de la toma de imágenes, cuando se definan los campos tocara a un físico medico el cálculo de la distribución de dosis, a nivel de la lesión o lecho tumoral y de los órganos vecinos y finalmente se iniciara el tratamiento y se lo realizara de forma continua; la administración del tratamiento en los equipos estará a cargo de los técnicos de radioterapia.
En la braquiterapia el médico especialista realiza la colocación de aplicadores en el lugar donde se ha originado la lesión tumoral, ayudado por una enfermera, se toman imágenes de verificación y se realiza el cálculo de distribución de dosis por un físico médico, se trata normalmente por varios minutos y se retira; se realiza de forma semanal, de 3-4 aplicaciones en total.

¿Por quienes está formado el equipo de radioterapia?

- Oncólogo Radioterapeuta: médico especialista en radioterapia, está encargado de la prescripción de la dosis del tratamiento, planificación, controles del paciente durante el tratamiento y seguimiento al concluir el mismo.
- Físico medico: responsable del cálculo de dosimetría o distribución de dosis a nivel de la enfermedad y tejidos sanos.
- Enfermera de radioterapia: asistencia junto al médico durante la consulta y los diferentes procedimientos de radioterapia, orientación a los pacientes sobre los cuidados necesarios durante la radioterapia.
- Técnico de radioterapia: atención de los pacientes en el equipo de radioterapia durante todo el tratamiento, participación en la planificación en la toma de imágenes.
